Your patch will be much weaker if you only span one joist bay. Cut out enough subfloor to span at least 2 if not 3 bays (3 bays=4 feet). The patch will not flex nearly as much if you have done this. Placing some 2x4's perpendicular to the bays (parallel to the patch) will help avoid more flex, but isn't necessarily a have-to.

Yes, sistering the 2x4's to the joists to have something to nail to is good, make sure you have it "screwed-glued-and-nailed".

What caused the water damage? I assume you have fixed that...
