I have never seen an access panel inside the surround. And plumbing does not usually run in exterior walls - so I'm assuming you could cut the access hole in the room that is behind the bathtub (& then frame it in nicely.)

If you can't access the plumbing from *behind* the tub, then it's really not practical. (i.e. - it's easier just to knock a hole in the wall in the rare cases when you need access, rather than to create a watertight access panel inside the wetwall.)
