If you have really hard water and you're not using a water softener, the bottom of the water heater tank could be filled with lime etc. which prevents the water from getting hot. Another problem that I've heard of is that the pilot for some reason tends to go out easily.

I have an AO Smith glass lined and my brother replaced the pilot with one from an oil burning furnace because of this problem.
