pull off the base shoe if there is one. snap a line at 45* or whatever angle you choose, roughly through the center of the room, or wherever you can set the longest line. start your pattern by laying your tile along this line. lay all your tiles whole row at a time working outward from your center row. All the full tiles first, then do all your cut pieces, then reinstall the base shoe. Its really no harder than laying it square to the room, and in one sense easier, since there is no concern for out-of-square walls interfering with your pattern
