six wires, only two used - help!

My fax line (which a totally separate line) went dead on me. When I called the phone company they had me check the outside network box and it's fine, so the problem is in my inside wiring. I decided to replace the phone jack. The way the old jack was hooked up when I removed it was with only two wires, the black and the blue. The yellow and the white were capped off, and the green and red were just hanging.

My question is, how do I relate that to the standard red,green,yellow and white? I've tried just about every combination I can think of, and I'm going nuts! Am I making any sense? Thanks for any help you can give me.
