No need in going to all the trouble of tearing-out old grout, I wouldn't wish that on my worst enemy!! As mentioned before, use chlorine bleach, and plenty of it, thick rubber gloves, and a hearty scrub brush, with thick rigid bristles, not one of those wimpy numbers with nylon bristles. It's a tough job, but with the proper tools, you have a shot, and it's much easier than tearing anything out!! The hardest bristle brush you can find, would be great, as long as it doesn't scratch the tile. Saturate the counter in bleach, then more bleach as you scrub, you will see the years of build-up drawing out of the 'porous' grout. After it is thoroughly clean, and all traces of the bleach are neutralized with water, and your countertop has had a few days to dry, then go to your local area home center, pick-up some grout sealer and follow the instructions. After they are sealed, reseal them every 6-8 months, this will keep them looking fresh and will keep the grime from penetrating the pores!!
