removing paint

The paint is flaking off. Mostly on the ceiling above the bathtub. We had sanded and scraped most of the original oil base paint off and sanded the primer that was left before we painted with latex primer and then with latex paint. That was three years ago, it started flaking within a year. We thought we had done a good job of prepping it, obviously not. Now I would like to do it the right way. Another question, Can we use oil base primer and then latex paint when we paint it again? I'm trying to avoid the odor of the oil base paint. I know oil base paint would be the best in the bathroom. Also, what brand name paints would you suggest? Thank you for all your help.
