When you said "connected to an electrical outlet" do you mean that they yellow and black wires are hooked up to the electrical outlet as part of the electrical circuit? If so, that is very strange and likely dangerous, and you should remove that connection.
As far as wiring up a new jack, there's nothing real tricky about it. The new jacks usually have the wire colors stamped on the backs of them to show you what color wires go to which screws.
