Old (very old) ceiling fan switch replacement

This is one of the originals, the cast-iron type. It stopped without worning, and when I investigated I found the
'pullchain" switch had burned up and when I removed it, none of the original wires remained on the switch.
I have a blue wire, a red wire, a white wire, a black wire and a white/black wire (which I know to be the ground in this case.)

Can you help me put humpty-dumpty back together again...wiring-wise, that is. I will be controlling the light and fan with one wall switch, and turn the light on/off with the pull chain.
