Cheryl, you should use some sort of threshold to span the wood and ceramic. That wide of a grout will do this all the time. You are getting movement between the wood and the ceramic which the grout cannot handle. Any slight movement will cause it to crumble every time.

Get a special threshold found at a home center that will span the wood and go over the lip of the ceramic tile. Will look nice and eliminate your problem.
