New concrete over old concrete

I have a patio that is 5" below the main slab. I want to pour concrete over the old to build an addition on. The patio has several cracks that go all the way through. Is it a good idea to pour new over old or should the old come out. Could a layer of sand over the old concrete prevent reflect cracks. Rebar epoxied into the vertical portion of the house slab should hold the new slab in place. Any help or advice will be appreciated. Thank you
