Need help with touch dimmer switch

I have an old lamp in which I installed a touch dimmer switch. During a move, the wiring came undone, and I can't figure out how to rewire it. The dimmer is a Westek Touchtronic Light Control, model 6503, and it has 4 wires: black, gray, brown, and yellow. The yellow wire is still attached to the lamp itself, and the the black wire is attached to one of the wires in the plug cord. That leaves three wires (two to the bulb and one to the plug) to attach to the two remaining wires on the dimmer. Can anybody help me figure out how to hook everything up? Thanks!
