both of these problems are generally created by lack of proper ventilation. international residential code states a minimum of 1sq ft of ventilation for every 150 ft of area and vents should be placed with in 3 ft of the corners.but other factors that may be leading to this condition should be investigated- if the crawl space has a dirt or gravel floor;it should have a vapor barrior, either visquene or 6 mil. plastic.another cause could be the lack of gutters with downspouts & leaders that direct rain water away from the house/ or improper grading that is allowing water to flow against the house -- water is a homes greatest enemy.
