insulating a crawlspace

I would appreciate any help with this. We have a kitchen with a crawlspace below. The floor is always cold. The attic seems to be poorly insulated. I would like to know the best insulation size and method to use. Should I put down plastic on the ground? Do I use faced or unfaced? Should I seal the two vents where the cold air enters? I have 2 x 10 joists. What is the proper R value insulation to use? We are new to this and any help would be appreciated.
