Insulating a crawl space

I want to have the crawl space area in my home insulated (yes, believe it or not it never was). I understand that there should be a plastic sheet for moisture control. Does the sheet lay directly on the ground with the insulation between the joyce (is that spelled right?)?

What thickness of plastic sheeting, please, and if it lies directly on the ground how is it held there?
