I've seen this done several ways. Most of the time Carpenters nail through the face of the board, just as Steve says. A Carpenter I worked with in MI used to nail furring strips onto the wall in a horizontal direction and cut the wood into random sections, bevel the butt end with a plane and assemble them in a vertical direction, nailing through the tongue by pre-drilling each hole.

The finished product was stunning. But I would think that this is not a procedure for the beginner or the faint of heart. I'd go with Steve on this one.
