When I built my house, I was advised by a "third-generation floor man" to use plastic sheeting for a moisture barrier, and full-coverage was emphasized. I then nailed down 7/8" plywood on 16" centers using a hammer drill and concrete nails (you can use the power drivers available). The the floor guy put down black paper as another moisture barrier (and he said it quitened the floor) before nailing down the oak. I used traditional 3/4" red oak.
