grouting floor tile

I've recently completed grouting 12" floor tile. The tiles are spaced 3/8" apart. I used sanded tile grout and followed instructions on the grout carton. The problem is that in certain areas of the floor the grout is cracking and flaking. Do you know what causes the grout to crack and flake? Thanks.
