Go with the prebuilt, fold-away. They're easy to install (i've put in 2 so far, and am planning a third {split level home}) Before I broke down and bought a prefab, I research getting a custom built one. Trust me, they're way to expensive and scarce. (unless you've got a historical home)
Typically, the prefab fold-aways run about $60-$80 U.S. and come in a variety of lengths. If you have the option, you want to install the stairs in the SAME direction as the ceiling joists. You can install either with or across the joists, but obvoiusly installing across the joists involves more cutting, reinforcing and time.
Good luck, should take you about half a day, and you'll need a helper at some point to lift the whole thing in place.
