I found this website that may be helpful not sure but check it out...
locate the sears remote that resembles yours and click on the technician button by the compatible liftmaster remote...it has instructions on how to program the remote.
If this is not helpful...you should call Sears parts and service and see if they have a craftsmans website with the instruction s for programming your remote that you can download and print out...otherwise ask them to send you a new set of instructions...they should be willing to do so for free. hope this helps...post back with what you ended up doing to get your instructions.
