Excessive Window Condensation

I have a fairly new home (2 1/2 years old) in a suburb of Chicago that we built via a local builder in the mid to upper price range.

We have experienced excessive window condensation every winter when the temperature drops below approx. 20 degrees, which is quite often! We were told condensation was normal for the first year or so as materials dry out, but it's definitely time that this should be going away. It isn't. Now the paint/wood is getting damaged.

I've been told the house is possibly too "tight" and may need an air exchanger. The builder says he has never seen this before and doesn't know what it is.

Can anybody tell me what might be causing this? Could it be caused by faulty window installation? I would greatly appreciate any suggestion or comments.
