I just did this. If you buy a new phone jack plate, there are usually instructions on the back of the package. Basically, strip the outer covering of the brown wire a couple of inches to expose the red, black, green, yellow wires (I don't know if 3 is standard, but mine had 4). Then attach each wire under a screw in the phone jack. Usually they are color-coded for each wire. Tip: do this *before* you attach the phone jack completely to the wall. Test and make sure it works before you screw it into the wall. Once you attach the individual wires and test, attach the plate to the wall and you're home free. Hope this helps.
