There should be a warranty on this work. Go after your contractor to fix what was not done properly in the first place.
There should have been all of those spcifications in your driveway if not more. Also, it could have been a "bad" pour of concrete. Meaning that it was not the proper mixture of water, Portland cement, or sand.
Preparation is usually two-thirds of any job, and definitely should have been in your driveway.
If you don't get any recourse from your contractor, try to either go to the local media to shed some light on "the local contractor that does bad" story or get an attorney. If you are in a cold climate then you really have to get this resolved before it breaks apart into a gravel driveway.
