CONDENSATION BETWEEN INSULATION & VAPOR BARRIER

We are finishing off our basement. In setting my walls next to the foundation, I placed a plastic vapor barrier next to the concrete foundation inside the wall. I then
placed rolled insulation within the wall. Soon after, condensation and ice formed between the plastic & the insulation. What can I do to fix my problem? I have been told
possible remedies include 1) placing an additional barrier over my insulation before installing drywall 2) Tear out the inside vapor barrier between concrete foundation & the inside
wall.
