concrete parking spot

Hi All
I need to put in a concrete slab for a parking spot and am getting conflicting information from various people. I need to know how deep should the base be. What material should the base be (I have gotten answers from crushed stone, to limestone, to sand - I don't think the last two are right) and how thick should the slab be. I'm also getting conflicting info on whether I'll need rebar for this project. This will be for a 12 x 13 slab total. My plan was to dig down about 6", level the ground, add 4" of stone, compact it and then add about 2" of conrete, with proper expansions joints, etc. Any and all info and corrections to my plan will be appreciated. Thanks for your help
