Don't know anyone who rents their concrete forms out to one time users. Forms can ruined if not cleaned properly most who have invested in them don't want to take a chance. Most of the prefab forms I've used are either steel framed with a plywood face or aluminum framed with a plywood face. If you are looking at a one time use I think buying would be cost prohibitive and you could probably have the job done for less than the cost of the forms. It's fairly easy to make 2X4/plywood forms that will hold up over several pours. If you describe what you're trying to do maybe I can help.
