Concrete Driveway

I am trying to kill two birds with one stones. I have some leakage on one side of the house. My current driveway consists of two rows of pavers. I also experience basement leakage on that part of the house. From some small testing, I have discovered that if I were to replace the two rowed pavers with an actual driveway I may have a much drier basement. I am wondering if someone could tell me the basic setup for such a construction. I understand the basics of it that it requires a solid underbase, etc... but could some one tell me the entire layer process... Sand, class rock, concrete. I have also heard that it might be wise to lay a plastic protective cover down first before the rock/sand or whatever.
