Ceramic Tile - Floor Preparation

I have a linoleum floor on 3/4" plywood subfloor in my bathroom. I want to install ceramic tile. Do I need to remove the linoleum? Is the subfloor thick enough? If I have to put down more plywood/tile backer the floor will be too high for the existing toilet connection. What do I do about that? What about the transition from the tile floor to the carpeted hallway? Is the baseboard usually removed prior to installing a new floor? Any suggestions for getting it off without damaging the drywall/wallpaper?
