Ceramic Tile & Tub Plumbing

I have two questions:

First, I have removed the vanity, toilet and tub in the bath and would like to know should ceramic flooring be layed while all this has been removed and installed wall to wall? Or, should the vanity, toilet and tub be installed and then the ceramic tile layed around them??

Second, I have purchased a two piece fiberglas tub/shower and realized after the purchase that the existing plumbing and drain will be off approximately 3/4" so that the drain and overflow will not line up, is there any easier way of lining this up other than moving all the plumbing over 3/4"??
