asphalt driveway fixing

The asphalt driveway on my 60 yr. old house is rutted, lumpy and cracked, so I think it needs to be totally replaced. Two local paving contractors are coming later this week to give me estimates, but I would like some background information on this type of work, so that I can ask the right questions and keep to my budget. I know some of your shows covered basic concepts, but can you provide me with (or refer me to another source) a general overview? How deep should they dig? What materials form the base (below the asphalt)? Is there a rough estimate of what it should cost (per yard) to tear up the old asphalt, regrade, etc. and put new asphalt down?
