Henry gave some good advice on a step by step approach. But I usually go to the final step as I don't like plaster and extra thick walls. One room at a time, I just tear off the plaster and lathe (very messy), install insulation and fix air leaks, and then put on new drywall so it is almost like new. Replace any bad windows at the same time with new ones.
