will insulating under floors in crawlspace stop dampness?

Is it better to use insulation with a vapor guard or with out a vapor guard? Which way should the vapor guard face? I am losing alot of heat through my floors which is making my crawl space warm which I believe is causing my crawlspace to stay damp and moisture coming up from the crawlspace and making the inside of the house humid and making some of the rooms have a musty smell. I have 6 mil plastic covering the whole ground in my crawlspace, underneath the plastic is where it is wet and where the ground meets the foudation, is damp. Gutters look good, I water proofed foundation, added foundation vents, buried a perferated tile and the ground slopes away from the house, also there are no plumbing leaks that I can see.
